SMART (Substance Misuse Assessment Referral Team)

About the scheme:
The Substance Misuse Assessment Referral Team (SMART) was launched on May 1st 2004.

The purpose of SMART is to simplify referral pathways into treatment / support agencies, to ensure that potential clients are placed with the appropriate agency to best meet their needs and to improve communication between referrer and agencies.

Who is the scheme suitable for?
SMART is accessible to anyone living in the Neath Port Talbot area who is experiencing problematic substance misuse, be that alcohol or drugs.

Support Service:
SMART will undertake a brief triage assessment on clients and provide support in the form of written information, brief intervention and signposting in order to address immediate needs of clients.

Referrals:
Clients can be referred by any health, social service professional, voluntary agencies or any other agencies.
